it's because you used the save file that already had the maximum hearts/stamina. there is probably a separate value in the files that disables upgrading completely once you reached the maximum. just make a fresh save file and edit that instead. or just use the editor to manually give you the upgrade. You cannot. I got as far as two hearts short of a full second row. I think before you could only get as far as three hearts short. This probably means that the champion shrines get you spirit orbs as well, but only so many of them.
